Medical: choosing the components

Why does supply longevity matter more in medical than anywhere else?
Because requalification is expensive. A medical device is certified with its exact BOM, and a discontinued component can trigger regulatory work far beyond the part cost. Choosing manufacturers with long-availability commitments, and a distributor who forwards every change notice, is part of the design.
What does patient isolation require from the components?
Defined means of patient protection — isolation barriers with specified working voltage, creepage and clearance, usually two independent means. That drives the choice of isolators, optocouplers, supplies and even connector spacing, and it is decided at architecture time, not at layout.
